At Pictou County Community Health Centre (PCCHC), we know that navigating the health and social service system can be overwhelming. That’s why our Support & Navigation team is here to make sure you don’t have to do it alone.
Whether you’re trying to book a medical appointment, apply for government programs, or find community services, our team will help you figure out the next step and connect you with the right people and services.

Our Support & Navigation team offers one-on-one guidance for:

Filling out government forms and applications

Virtual Care Access
Making virtual health care casy
Accessing healthcare shouldn’t be complicated, and with Pictou County Community Health Centre (PCCHC), it doesn’t have to be. Our Virtual Care Navigation program helps you get connected to doctors, nurses, and other healthcare providers online, even if you’re not sure where to start.
If you’re new to virtual care, don’t worry. We’ll walk you through it step-by-step, making sure you feel comfortable and confident using the technology.
What we can help with
Our Virtual Care Navigator can assist with:
Setting up an account for Virtual Care NS
Booking online medical appointments, including bloodwork
Troubleshooting connection or technology issues
Explaining how to use your smartphone, tablet, or computer for care visits
Providing access to private, reliable internet for your virtual appointment (if needed)

At Pictou County Community Health Centre (PCCHC), we believe that good care doesn’t end when you walk out the door. Our Follow-Up Care service makes sure you’re supported every step of the way, after your appointment, after your referral, and until you’ve received the help you need.
We know that taking the next steps after a visit can feel confusing or overwhelming. That’s why our team follows up personally to make sure nothing falls through the cracks.
How we support you
Our Follow-Up Care team will:
Check in after your appointment to see how things went
Confirm that referrals or prescriptions have been received and understood
Help you book follow-up appointments or additional care
Provide reminders for upcoming visits or tests
Offer help connecting to community programs or ongoing supports
